In healthy people, thyroxine is converted into the biogically active thyroid hormone called triiodothyronine, or T3. T4 production is controlled by thyroid stimulating hormone or TSH, a hormone produced by the pituitary gland. Blood tests to measure TSH, T4 and T3 are readily available and widely used.

The thyroid consists of two hormones known as triiodothyronine (T3) and its prohormone Thyroxine (or T4). T4 is a prohormone which must convert to the active T3 hormone for the body to respond. T3 is the bioactive thyroid hormone that controls the important functions related to thyroid hormone balance. Some people do not adequately convert enough T4 into T3 and therefore still experience symptoms of hypothyroidism despite showing normal T4 levels. Achieving optimal T3 levels is an important outcome necessary for the effective treatment of hypothyroidism and related symptoms.

Levothyroxine (T4) or L-thyroxine is a synthetic thyroid hormoen that is chemically identical to thyroxine (t4), which is naturally secreted by the follicular cells of the thyroid gland. It is used to treat thyroid hormone deficiency, and occasionally to prevent the recurrence of thyroid cancer.
